I hereby state that the above-named Rower has permission to participate in Haddonfield Crew and will abide by all Rules and Regulations of the Athletic Department and attend all practices which are mandatory unless otherwise excused by the Coaches.

I understand and agree that Haddonfield Crew may occasionally use photos of athletes and parents as part of its website, social media, or marketing materials.

I understand that I must complete two additional steps for my rowers to be cleared to practice: (1) register for Crew through Genesis and (2) submit all medical forms to the school nurse.
